Locally nurse and family owned, Take Care Home Health was established by Susanne S. Wise, R.N., M.B.A., to meet the need for a private duty home health care agency that would uphold higher standards and set precedence for care in our local community.

Take Care’s legacy of success, over 24 years and five offices later, is in large part due to Susanne’s continued intimate involvement, such as her appointment of senior leadership who understands and supports her original and continued crucial elements of success: time and people.

Susanne’s daughters, Courtney and Erika, provide continuity of character and consistency of values, and contribute a wealth of education, experience and individual community leadership that propel Take Care forward.
